This asthma log has been designed for use within the education sector such as for schools colleges and nurseries. 

The log contains a range of information on how to deal with an asthma attack as well as a range of forms to record information such as which parents have provided permission for an emergency asthma inhaler to be used for their child, logging when an emergency asthma inhaler has been used and when the asthma inhaler kit has been checked.
